POSITION SUMMARY:

The Targeted Case Manager plays a crucial role in enhancing the quality of life and success of clients in long-term recovery. This position involves creating individualized, goal-oriented support plans aimed at fostering self-sufficiency. The case management process is dynamic, encompassing assessment, planning, implementation, coordination, monitoring, and evaluation to achieve client-centered objectives.

KEY RESPONSIBILITIES:

  • Conduct assessments for new clients and formulate care plans
  • Implement care plans consistently and efficiently
  • Collaborate with team members across Clinical, Programs, Education, and Aftercare Departments
  • Assist clients in accessing medical, social, educational, and vocational resources
  • Facilitate various aspects of care coordination and information sharing
  • Act as an advocate for clients, guiding them in making informed decisions about their treatment
  • Proactively engage with clients to document their progress and ensure successful outcomes
  • Maintain accurate records of assessments, plans, notes, and discharges
  • Comply with Kentucky Administrative Regulation (KAR) standards and regulations

QUALIFICATIONS:

  • Bachelor's degree in a behavioral science or related healthcare field
  • At least one year of post-graduate experience in a healthcare or human services setting
  • Strong written and verbal communication skills
  • Excellent organizational and time management abilities
  • Preferred experience as a Targeted Case Manager in the substance use disorder field
  • Familiarity with case management principles and healthcare management
  • Knowledge of Medicaid regulations is advantageous
  • Effective problem-solving skills and the ability to manage multiple tasks
  • Collaborative mindset with a process-oriented approach
  • Current TCM (CORE/SUD) certification is a plus
