We are seeking a highly skilled and compassionate Primary Care Nurse to join our team at Providence Transitional Care Center. As a Primary Care Nurse, you will be responsible for providing high-quality patient care and leading a team of healthcare professionals to deliver exceptional outcomes.
Key Responsibilities- Provide direct patient care, including assessment, planning, implementation, and evaluation of care plans.
- Supervise and mentor a team of nurses to ensure high-quality patient care and adherence to established standards and protocols.
- Collaborate with interdisciplinary teams to coordinate patient care and develop effective care plans.
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care transitions and optimal patient outcomes.
- Participate in quality improvement initiatives to enhance patient care and safety.
- Graduate of an accredited School of Nursing.
- Current Alaska Registered Nurse License.
- National Provider BLS - American Heart Association certification.
- 6 months Nursing experience in a healthcare facility (acute care, long-term care, rehabilitation center).
- Supervision/leadership experience.
