About the Role:
The Clinical Pharmacist collaborates with a multi-disciplinary team to ensure the delivery of safe and high-quality infusion therapy to patients in alternative care settings.
This role involves assessing and monitoring medication therapy in relation to the comprehensive needs of the patient while working closely with physicians, nurses, and other healthcare professionals to achieve optimal outcomes.
Schedule:
Monday - Thursday, 8:00 AM - 5:00 PM, with rotating on-call responsibilities.
Minimum Qualifications:
Experience/Education:
Bachelor of Pharmacy
Active pharmacist license
Clinical pharmacy experience from prior employment or clinical rotations
Proficient in essential software applications (e.g., MS Outlook, Word, Excel).
Ability to adapt to a dynamic and evolving environment while maintaining composure under pressure.
Must be able to function effectively as a team member and cultivate productive and cooperative relationships with all members of the Palmetto Infusion/AccuRX team.
Essential Functions:
1. Reviews, receives, and verifies all patient orders and prescriptions, documenting in accordance with policy, procedure, and applicable pharmacy laws.
2. Participates in the direct oversight of Pharmacy Technicians, Pharmacy Support Technicians, and Pharmacy Logistics Technicians.
3. Engages in on-call rotation for after-hours support for home infusion or ambulatory infusion services.
4. Maintains required competencies based on assigned responsibilities.
5. Ensures active licensure and stays current with all necessary continuing education.
6. Acts as a drug information resource for internal staff and external referral sources.
7. Provides comprehensive counseling to patients who seek pharmacy consultations.
8. Reviews clinical data, including relevant lab results and patient information, to ensure safe medication provision.
9. Adheres to all federal and state pharmacy regulations.
10. Clarifies and processes orders from physicians or authorized medical agents.

Physical Demands:
