About the Role:

We are seeking a highly skilled and empathetic Nurse Care Coordinator to join our Fairview Partners Care Coordination team. As a key member of our team, you will play a vital role in ensuring that our members receive high-quality, patient-centered care.

Your primary responsibility will be to coordinate care for seniors in northern Twin Cities metro and neighboring rural communities. You will work collaboratively with healthcare providers, family members, and community resources to develop and implement personalized care plans that meet the unique needs of each individual.

Key Responsibilities:
  • Assess the needs of our members and develop comprehensive care plans that promote independence and quality of life.
  • Collaborate with healthcare providers, family members, and community resources to ensure seamless transitions and continuity of care.
  • Maintain accurate and up-to-date records of member interactions and care plans.
  • Communicate effectively with members, families, and healthcare providers to ensure that everyone is informed and engaged in the care process.

Requirements: Bachelor's degree in nursing or equivalent, 1-3 years of clinical nursing experience, and a Minnesota Board of Nursing RN license in good standing. Preferred qualifications include a Public Health Nurse license, certification in case management, gerontological nursing, or public health nursing, and basic computing skills.

This is a full-time position with an estimated salary range of $65,000 - $85,000 per year, depending on experience.
