About the Role

We are seeking a skilled Behavioral Health Care Coordinator to join our team at CVS Health. As a key member of our organization, you will play a vital role in facilitating appropriate member physical health and behavioral care.

Key Responsibilities:
  • Conduct comprehensive assessments of member needs and eligibility by evaluating benefit plans and available services.
  • Develop strategies to reduce risk factors and address complex clinical indicators that impact care planning and issue resolution.
  • Perform crisis intervention with members experiencing a behavioral health or medical crisis, refer appropriate providers, and ensure members receive necessary services.
  • Counsel medical and behavioral health professionals to influence member care.
  • Coach members to make independent healthcare decisions and participate in care planning.
Requirements:
  • New Jersey License: LCSW, LMFT, LPCC.
  • Resident of New Jersey.
  • At least 3 years of experience in behavioral health and social services.
  • 2+ years of experience with personal computers and MS Office Suite applications (Outlook, Word, Excel, SharePoint).
  • Ability to travel up to 25-50% with mileage reimbursement.

Estimated salary for this position is $65,000 - $85,000 per year, depending on location and experience.
