We are seeking a skilled and compassionate Home Health Registered Nurse to join our team at Interim HealthCare. As a Home Health RN, you will provide high-quality patient care in the comfort of their own homes, working closely with a multidisciplinary team to ensure seamless care transitions.
Key Responsibilities- Provide comprehensive home-based care to patients with various medical conditions, including illnesses, injuries, and chronic diseases.
- Collaborate with physicians and oversee the care provided by LPNs, CNAs, and aides.
- Conduct thorough patient assessments, document progress, and report changes to the patient's physician.
- Develop and implement personalized care plans, educating patients on their treatment, medication, therapy, diet, and exercise.
- Coach family members on patient care, medication, and home safety.
- Ensure patient goals are met and coordinate discharge from services.
- Active Registered Nurse (RN) License in Colorado.
- Graduate of an accredited school or program of nursing.
- At least 1200 hours of experience as an RN.
- Healthcare CPR/BLS Certification (demonstration course required).
- Home health care experience preferred.
- Pass a background check and drug test (we are required to test for THC).
