This full-time position is located in Middletown, RI, Fairhaven, MA, and West Kingston, RI, with a 40-hour workweek. As a Nurse Practitioner, you'll work closely with our post-acute therapy team to optimize rehabilitative outcomes and promote patient independence.
Responsibilities:- Provide specialized PM&R/Physiatry services as a rehab consultant.
- Represent CRC in your facility through strong clinical leadership and collaboration with the interdisciplinary care team.
- Collaborate with PCP teams to help manage conditions common in SNFs such as CVA, progressive neurological conditions, and postoperative orthopedic surgeries.
- Nurse Practitioner/APRN/ Physician Assistant license.
- Rhode Island License.
- Ability to work in the U.S.
- Experience in post-acute space preferred.
