Patient Care Specialist

We are seeking a dedicated Patient Care Specialist to join our team at Grand Itasca Clinic & Hospital. As a Patient Care Specialist, you will be responsible for conducting focused assessments, participating in patient health plan development, and implementing patient care interventions delegated by licensed healthcare providers.
Key Responsibilities:
- Conduct focused assessments and report changes to licensed healthcare providers
- Participate in patient health plan development and modification
- Implement patient care interventions delegated by licensed healthcare providers
- Assign and monitor nursing tasks to certified medical assistants or unlicensed personnel
Requirements:
- Current LPN Minnesota state license or current Medical Assistant certification/registration required
- Experience in an ambulatory healthcare setting preferred
- Ability to work efficiently, accurately, and maintain positive working relationships
- BLS certification within two months of employment
- Demonstrated ability to follow and adhere to Grand Itasca Clinic and Hospital policies, vision, mission, and values
- Maintains reliable and consistent attendance
