Job Title: Home Health Physical Therapist

Mission Healthcare is seeking a skilled Home Health Physical Therapist to join our team in National City, California.

About the Role

We are looking for a highly motivated and experienced Physical Therapist to provide high-quality physical therapy services to patients in their homes. As a Home Health Physical Therapist, you will be responsible for assessing and evaluating patients' physical therapy needs, developing and implementing individualized treatment plans, and providing ongoing care and support to patients and their families.

Responsibilities
  • Assess and evaluate patients' physical therapy needs and develop individualized treatment plans
  • Provide physical therapy services to patients in their homes, including exercises, stretches, and other interventions
  • Conduct home safety assessments and provide education to patients and their families on safe home environments
  • Collaborate with other healthcare professionals to ensure comprehensive care and coordination
  • Document patient progress and outcomes in accordance with Mission Healthcare's policies and procedures
Requirements
  • Master's degree in Physical Therapy from an accredited program
  • Current licensure as a Physical Therapist in the State of California
  • At least 2 years of experience as a Physical Therapist, preferably in home health or community-based settings
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
