Position Overview

The Registered Nurse (RN) in hospice plays a vital role as a proactive member of a multidisciplinary team, delivering tailored, high-quality, and safe care to patients and their families during the end-of-life journey. This role involves providing direct patient care in various clinical settings within the hospice and associated healthcare facilities, guided by Nurse Managers and other leadership personnel. The practice framework is informed by the WellStar Professional Practice Model, 5-Star Nursing, and evidence-based practices and research. RN competencies are aligned with these models and supported by the WellStar Core Competencies.

Key Responsibilities and Essential Functions

1. Conducts initial presentations of hospice services.
2. Articulates the palliative care approach and evaluates patients' care objectives, including their wishes regarding Do Not Resuscitate (DNR) orders. Discusses the benefits of hospice services provided by payers and clarifies the financial responsibilities of patients. Secures all necessary consents and election statements while determining the patient's preferred attending physician. Develops the initial Plan of Care.
3. Delivers direct nursing services to terminally ill patients and provides supportive care to their families, with compassion and sensitivity to their physical, social, cultural, and spiritual needs.
4. Implements the Nursing Process, which includes assessment, diagnosis, outcome identification, planning, implementation, and evaluation. Provides relationship-centered, patient-focused care that is consistent with the specific characteristics of the population served (e.g., age, gender, disease progression) while respecting cultural diversity.
5. Collaborates with patients and their significant others, ensuring they are informed and involved in the care plan.
6. Maintains accurate documentation of all care provided in the Electronic Medical Record (EMR) according to policy timelines.
7. Offers ongoing education to patients, caregivers, and family members regarding disease progression and anticipatory grief.
8. Provides emotional and spiritual support to patients and families, fostering a safe and peaceful environment.
9. Assesses patient symptoms and medication responses, ensuring prompt action to maximize patient comfort.
10. Coordinates the delivery of safe, quality patient care through effective interdisciplinary communication and documentation.

Professional Development and Support

1. Completes all required competency assessments and ongoing education.
2. Serves as a mentor for other nursing professionals, fostering a qualified workforce and a healthy work environment.
3. Participates in data collection and evidence-based practice initiatives to enhance patient care outcomes.

Qualifications

1. Graduate of an accredited nursing program.
2. Bachelor’s degree in nursing.
3. Required Licenses and Certifications:
- Registered Nurse (Single State) or RN - Multi-state Compact
- Basic Life Support (BLS) certification.
4. Minimum of acute care experience as an RN.
5. Strong communication, interpersonal, and critical thinking skills, with the ability to function effectively in a fast-paced environment.
