Job Summary:

The Social Services Specialist at Carle Health identifies the psychosocial needs of patients and families through assessment. This role involves providing resource support, short-term counseling, and crisis intervention as part of an interdisciplinary team. The ideal candidate will have a Master's or Bachelor's degree in Social Work and be licensed as a Social Worker (LSW) or Clinical Social Worker (LCSW) in Illinois.

Key Responsibilities:

  • Conduct psychosocial assessments to identify emotional, social, and environmental strengths and problems related to patient diagnosis, illness, treatment, and life situation.
  • Develop and implement social work plans to enhance patient and family coping skills, support cultural beliefs and values, and maximize medical benefits.
  • Collaborate with interdisciplinary teams to provide comprehensive care and support.
  • Document patient and family assessments, interventions, and outcomes in the electronic medical record.
  • Facilitate patient decisions and communication of self-determined life care decisions.

Requirements:

  • Master's or Bachelor's degree in Social Work.
  • Licensed Social Worker (LSW) or Clinical Social Worker (LCSW) in Illinois.
  • Clinical experience in hospice care.
  • Ability to work as part of a team and provide sensitive care to patients and families.
