Social Worker Medical Specialist

Job Summary: We are seeking a skilled Medical Social Worker to join our team at Carle Health. As a Medical Social Worker, you will play a vital role in providing emotional, social, and practical support to patients and their families.
Key Responsibilities:
- Develop and implement social work plans of care to address the unique needs of patients and their families.
- Provide counseling and crisis intervention services to patients and their families.
- Assess and address the social and emotional needs of patients and their families.
- Collaborate with the interdisciplinary team to develop and implement comprehensive care plans.
- Document patient progress and outcomes in a timely and accurate manner.
Requirements:
- Masters or Bachelors Degree in Social Work.
- Clinical experience in hospice care or a related field.
- Strong communication and interpersonal skills.
- Ability to work effectively in a team environment.
