EMPLOYMENT STATUS: Full-Time Permanent

As a Patient Care Specialist, your primary role will be to facilitate care coordination for members enrolled in Medicaid, Medicare Advantage, and/or Dual Eligible Special Needs Plans (DSNP). You will strive to create a supportive environment for members by fostering trustworthy relationships. This involves assisting members in accessing appropriate healthcare services in a timely manner, collaborating with health plans, healthcare providers, pharmacies, and community organizations. You will assess and address both medical and social determinants of health (SDOH) for each member.

Qualifications:

  • Minimum of 2 years of experience in care coordination, case management, community health work, pharmacy assistance, or social work.
  • Fluency in English is required; proficiency in Spanish is a plus.
  • A dedicated home office setup for remote work is necessary.
  • Must be a resident of Mississippi.
  • Experience with Medicaid and/or Dual Eligible patients is preferred.

Essential Skills:

  • Excellent interpersonal, verbal and written communication, and problem-solving abilities.
  • Experience in customer interactions via phone and text messaging.
  • Proficiency with Apple devices (e.g., MacBook).
  • Experience in documenting case notes within care management or electronic health record systems.
  • Familiarity with motivational interviewing techniques.

Core Responsibilities:

  • Engage in outreach efforts to motivate and educate members about available programs.
  • Conduct assessments of members' needs and collaboratively develop care plans to meet their goals, primarily through phone and text communications.
  • Evaluate individual care requirements and relay medical information to healthcare professionals.
  • Manage a caseload to ensure timely contact with each member.
  • Facilitate care coordination with providers and assist in scheduling necessary appointments, encouraging members to actively participate in their health management.
  • Identify and address needs related to Social Determinants of Health.
  • Coordinate and complete correspondence in line with established workflows.
  • Document all actions taken accurately in a care management platform.
  • Conduct a high volume of outreach to members, families, providers, or other relevant parties as required.

Work Schedule:

  • 8-hour shifts during standard business hours, as per the assigned contract.
  • Five days a week, Monday through Friday.

Remote Work: This position is remote, but candidates must reside in Mississippi.
