The Opportunity

We're seeking a skilled Float Medical Assistant to join our team at our Lakewood, CO office, which serves our Senior Health member population (age 65+). As a key member of our primary care team, you'll play a vital role in supporting patients with their preventive, chronic, and acute care needs. Your primary responsibilities will include establishing trusting relationships with patients and their families, capturing relevant health information, and collaborating with providers to ensure seamless care coordination.

Key Responsibilities
  • Capture patient health information and vital signs, administer point-of-care testing, and perform standard age and condition-appropriate screening assessments
  • Assist with post-visit care coordination, scheduling appointments with specialists, and sharing information with the patient's care team
  • Collaborate with providers to monitor patient health and ensure up-to-date preventive measures
  • Participate in daily practice operations, including answering phone calls, responding to emails, and maintaining the clinical space
  • Provide patient education on chronic disease management and coach patients using motivational interviewing techniques
Requirements
  • At least 1 year of experience in a high-touch customer service or patient-facing role in a healthcare setting
  • Strong written and verbal communication skills
  • Experience working on collaborative, diverse, and feedback-driven teams
  • Proven track record of persisting through change, demonstrating a forward-thinking perspective, and consistently stepping up to take action on challenges
  • Proficiency in computer technology, including typing, navigating the internet, and using multiple software systems simultaneously
