About the Role

We are seeking a dedicated and compassionate Care Coordinator Specialist to join our team at Network Human Services.

Job Summary

The successful candidate will provide high-quality support to residents in a residential setting, promoting their independence, dignity, and well-being. This role involves providing direct care, managing daily routines, and maintaining accurate records of resident activities and progress.

Key Responsibilities

  • Direct Care: Provide personal care assistance, including bathing, dressing, grooming, and toileting as needed. Assist with activities of daily living (ADLs) such as eating, mobility, and communication. Foster socialization and development of meaningful relationships among residents.
  • Routine Maintenance: Participate in routine residence maintenance tasks, ensuring a safe and comfortable environment for residents. Report any concerns or issues related to the physical condition of the residence.
  • Safeguarding: Safeguard and properly handle residents' belongings and funds on the premises. Maintain confidentiality and respect residents' rights.

Requirements

  1. Qualifications: High School diploma or equivalent required; valid Connecticut Driver's License preferred.
  2. Skills and Qualities: Strong interpersonal and communication skills; ability to maintain confidentiality; proficiency in basic computer skills, particularly electronic health record systems.

Work Schedule:

This is a full-time position, requiring 38 hours of work per week. The schedule includes various shifts, including morning and evening hours, Monday through Thursday. Flexible scheduling may be required to cover unexpected absences.

Compensation: $45,000 - $55,000 per annum (dependent on experience).
