Key Responsibilities
  • Conduct thorough assessments of patients' health status and develop personalized care plans to meet their unique needs.
  • Collaborate with healthcare professionals to ensure seamless coordination of care and services.
  • Communicate effectively with patients, families, and caregivers to ensure informed decision-making and optimal outcomes.
  • Document all interactions and care activities in a timely and accurate manner.
  • Participate in team meetings and contribute to the development of care plans and strategies.
Requirements
  • RN degree from an accredited college or university, or a graduate of an accredited/approved LPN program.
  • Minimum of two (2) years of practical nursing experience, with three (3) years preferred.
  • Previous experience in a governmental waiver program or similar setting is a plus.
