Key Responsibilities
- Conducts initial and ongoing assessments to evaluate patient functionality and compliance with federal standards.
- Develops personalized in-home treatment strategies in collaboration with physicians and the healthcare team.
- Employs a variety of assessment and therapeutic techniques tailored to the physical and emotional needs of patients.
- Completes all necessary documentation for patient visits in accordance with regulatory guidelines.
- Manages and oversees the treatment and progress of an assigned patient caseload.
- Delivers high-quality, individualized care based on evidence and proven methodologies.
- Supervises therapy assistants and other relevant staff members as needed.
- Encourages and motivates patients throughout their rehabilitation journey to achieve optimal outcomes.
- Aims to minimize medication use and explore non-surgical alternatives.
- Monitors patient progress and communicates findings effectively to the healthcare team.
- Collaborates with physicians regarding any necessary adjustments to treatment plans.
- Prepares detailed reports on patient progress for physician review.
- Educates patients and their families on home exercise programs and daily living activities.
- Completion of a Physical Therapy program accredited by CAPTE.
- Preferred experience of at least one year in a hospital or home health setting.
- Bachelor's degree or higher in Physical Therapy.
- Current licensure by the Illinois State Board of Physical Therapy.
- Active AHA CPR certification is mandatory.
- Must possess reliable transportation, a valid Illinois driver's license, and vehicle liability insurance.
- Exemplary professional conduct and communication abilities.
- Strong interpersonal skills, adaptability, and professionalism.
- Ability to work independently while being a committed team player.
- Excellent organizational and time management skills.
- This position involves working in diverse settings, including professional offices, patient residences, and during travel.
- Regular use of standard office equipment such as computers, telephones, and fax machines is required.
- Must be prepared to work in various environments, which may include exposure to allergens and other conditions.
- Potential exposure to blood, bodily fluids, and infectious diseases is possible.
