We are seeking a compassionate and skilled Registered Nurse to join our team as an End-of-Life Nurse Coordinator. As a member of our hospice care team, you will provide expert nursing care to patients and their families, ensuring their comfort, dignity, and well-being.

About the Role
  • Develop and implement individualized care plans that address the physical, emotional, and spiritual needs of patients and their families.
  • Provide education and support to patients and their loved ones on end-of-life care, pain management, and symptom control.
  • Collaborate with healthcare teams to ensure seamless care coordination and effective communication.

The ideal candidate will have at least one year of clinical experience and be licensed in the state of practice. A valid driver's license, current CPR certification, and access to reliable transportation are also required.
