The Healthcare Coordination Specialist plays a crucial role in supporting the Care Team, which includes providers, nurses, medical assistants, and other healthcare professionals. This position involves:
- Facilitating care for patients scheduled for appointments and proactively managing care for those not on the schedule to ensure comprehensive preventive services for all patients within the assigned panel.
In this role, you will coordinate services for all patients, particularly those facing serious, complex, or chronic health issues, as well as those with psychosocial challenges.
Additionally, you will provide advocacy, information, and referral services to patients and their families to meet their medical and psychosocial needs.
Qualifications:
- Proficient in conducting assessments and employing assistance techniques.
- Knowledgeable in universal precautions, maintaining a safe workplace, and ensuring confidentiality.
- Experienced in health information management and accurate patient data documentation.
- Demonstrated clinical skills proficiency.
- Familiarity with medical charge reporting and record-keeping.
Key Responsibilities:
- Reviewing provider schedules and individual patient records to assist the care team in coordinating visits and future healthcare requirements.
- Managing non-appointment related inquiries from patients that pertain to care coordination and follow-up.
- Acting as an effective communication link between patients and medical staff, relaying messages and gathering necessary information.
- Utilizing registry data to inform care team members about preventive care needed for patients seen each day.
- Tracking all patients for follow-up and reporting purposes.
- Ensuring accurate data entry and tracking information in the PM and/or EMR systems.
- Regularly reviewing registry information for the assigned patient panel and arranging necessary care to proactively address healthcare needs.
- Collaborating with medical staff to provide case management services for patients with complex medical and/or psychosocial issues.
- Working with the medical team to develop and implement chronic disease management programs for conditions such as diabetes, asthma, congestive heart failure, hypertension, and depression.
- Coordinating care with pharmacies, insurance providers, and other community resources to ensure timely information flow.
- Maintaining up-to-date disease and registry data entry and utilizing registry reports to organize care plans for complex patients.
- Participating in team discussions regarding data requirements for effective panel management.
- Utilizing community resources efficiently to meet health and human needs.
- Providing backup support to other Care Coordinators or Care Team members as necessary.

Education and Experience:
A High School Diploma or GED is required. Graduation from an accredited practical/vocational nursing program is preferred. A Medical Assistant diploma/certification is also preferred but not mandatory.
A minimum of two years of clinical experience is required, ideally in a hospital or office practice setting with a focus on complex medical needs.
