Job Title: Nurse Practitioner - Family Medicine

We are seeking a highly skilled and compassionate Nurse Practitioner to join our Family Medicine team at Mercy Health's Perryville Clinic in Rockford, Illinois.

About the Role:

This is a full-time opportunity to work in a well-established practice, collaborating with an experienced physician to provide high-quality care to a diverse patient population. The ideal candidate will have excellent interpersonal skills, a passion for delivering personalized care, and a commitment to excellence.

Responsibilities:
  • Provide primary and preventive care to patients of all ages, from newborns to geriatrics.
  • Collaborate with the physician team to develop and implement patient care plans.
  • Conduct routine check-ups, diagnose and treat illnesses, and prescribe medications as needed.
  • Participate in ongoing education and professional development to stay current with best practices and advancements in the field.
  • Contribute to the growth and development of the practice, including participating in quality improvement initiatives and mentoring junior staff members.
Requirements:
  • Graduate of an accredited nursing program and a master's or doctoral degree in nursing.
  • Current licensure as a Registered Nurse (RN) and Advanced Practice Registered Nurse (APRN) in Illinois.
  • Current DEA number and any other required certifications.
  • Primary care experience and a strong understanding of family medicine principles.
