Job Summary

Key Responsibilities
  • Assess and manage complex wounds in accordance with established protocols and best practices.
  • Collaborate closely with interdisciplinary teams to develop and implement comprehensive care plans tailored to individual patient needs.
  • Educate patients, families, and caregivers on wound management techniques and prevention strategies.
  • Maintain accurate documentation of wound assessments, treatments, and outcomes.

Requirements
  • Valid Registered Nurse (RN) license in the state of New York.
  • Certification in Wound Care (CWCA, CWCN, WOC, WOCN or equivalent) required.
  • Minimum of 1 year of clinical experience in wound care, preferably in hospice or palliative care settings.
  • Strong communication skills and the ability to work collaboratively within a multidisciplinary team.
  • Compassionate and empathetic approach to patient care.
