Job Title: Clinical Nurse Coordinator - Patient CareEstimated Salary: $80,000 - $110,000 per year

Job Summary:

The Clinical Nurse Coordinator - Patient Care will play a crucial role in ensuring the smooth delivery of patient care services. This individual will work closely with healthcare providers to assess patient needs, develop effective care plans, and implement necessary interventions.

Responsibilities:

  • Evaluate and assess patients' health care needs based on established triage protocols and guidelines
  • Obtain input and direction from Providers as needed to determine best clinical course of action for specific patient health care
  • Accurately document patient symptoms and complaints, conduct nursing assessments, and follow policies, procedures, and protocols to ensure consistency and departmental effectiveness
  • Assist Providers with patient examinations, obtain and record patient's vital signs and weight, and perform medical procedures and wound care as needed
  • Administer and document medication administration and patient chart (oral and intramuscular injection), review Provider's orders, lab requests, and Providers Nurse buckets
  • Interview patients to determine medical problems and document in chart for Providers, assist patients with scheduling follow-up appointments, and complete other duties as assigned

Qualifications:

  • Graduated from an accredited School of Nursing as a Registered Nurse and one year of nursing experience in Community Health Center or Allied Health field
  • Strong customer service focus, effective verbal communication skills, teamwork orientation, organized, and able to manage competing priorities
  • Good judgment, resourceful, problem-solving, able to take and follow through with delegated tasks and accountability
