Job SummaryWe are seeking a dynamic Licensed Practical Nurse (LPN or LVN) to join our team. As a key member of our staff, you will be responsible for providing one-to-one care to patients in their homes, assisted living facilities, and skilled nursing facilities. You will work closely with our interdisciplinary team to assess client needs, develop personalized care plans, and deliver high-quality patient care that meets the highest standards of excellence.

Key Responsibilities
  • Conduct comprehensive assessments of patients' physical and emotional needs.
  • Develop and implement individualized care plans that address the unique needs of each patient.
  • Provide direct patient care, including medication administration, wound care, and vital sign monitoring.
  • Collaborate with other healthcare professionals to ensure seamless transitions of care and achieve optimal patient outcomes.

Requirements
  • Graduate of an accredited or state-approved nursing school.
  • Licensed as a LPN in the State of Washington.
  • A minimum of 1 year of experience in an acute care setting; home care or hospice experience preferred.

Estimated Salary: $32-$35 per hour. Benefits include medical/dental/vision insurance, 401(k) matching, short-term disability, health savings account, life insurance, PTO, pet insurance, mileage reimbursement, and a scrub allowance.
