CenterWell Home Health is seeking a skilled and compassionate Physical Therapist to join our team. As a Home Health Physical Therapist, you will play a critical role in helping patients regain their independence and manage chronic conditions or recover from injury, illness, surgery, or hospitalization.

About the Role

We are looking for a highly motivated and experienced Physical Therapist who can work independently and as part of a multidisciplinary care team. The ideal candidate will have excellent communication and interpersonal skills, with the ability to build strong relationships with patients, families, and healthcare professionals.

In this role, you will plan, administer, and evaluate physical therapy treatments and training programs for patients in their homes. You will also develop and implement individualized plans of care, incorporating evidence-based practices and patient-centered goals.

  • Key Responsibilities:
  • Conduct thorough assessments and evaluations of patients' physical abilities and functional limitations.
  • Develop and implement tailored treatment plans that address patients' specific needs and goals.
  • Administer and supervise physical therapy interventions, including exercises, stretches, and activities.
  • Collaborate with other healthcare professionals to ensure seamless care coordination and communication.
  • Document all interactions, assessments, and interventions accurately and thoroughly.

We offer a competitive compensation package, including an estimated salary range of $85,400 - $117,500 per year, based on location and experience. Requirements and Qualifications
  • Education and Licensure:
  • Degree from an accredited Physical Therapy Program (approved by the APTA)
  • Current and unrestricted Physical Therapy licensure
  • Experience and Skills:
  • Minimum of one year physical therapy experience preferred
  • Home Health experience a plus
  • Strong organizational and communication skills
  • Valid driver's license, auto insurance, and reliable transportation
