We are seeking a dedicated and skilled Physical Therapist to join our team at CenterWell Home Health. As a Home Health Physical Therapist, you will have the opportunity to make a meaningful difference in the lives of patients and their families.

About the Position

This per diem position offers flexibility and autonomy, allowing you to work on your own schedule and choose from a variety of patients and cases. You will have the freedom to develop and implement personalized treatment plans, using evidence-based practices and patient-centered approaches.

You will work closely with our interdisciplinary care team, including nurses, therapists, and social workers, to ensure seamless care coordination and communication. Your expertise and compassion will be invaluable in helping patients achieve their goals and regain their independence.

  • Responsibilities and Expectations:
  • Plan, administer, and evaluate physical therapy treatments and training programs for patients in their homes.
  • Develop and implement individualized plans of care, incorporating evidence-based practices and patient-centered goals.
  • Collaborate with other healthcare professionals to ensure seamless care coordination and communication.
  • Document all interactions, assessments, and interventions accurately and thoroughly.

We offer a competitive compensation package, including an estimated salary range of $85,400 - $117,500 per year, based on location and experience. Qualifications and Requirements
  • Education and Experience:
  • Degree from an accredited Physical Therapy Program (approved by the APTA)
  • Minimum of one year physical therapy experience preferred
  • Home Health experience a plus
  • Skills and Qualities:
  • Strong organizational and communication skills
  • Valid driver's license, auto insurance, and reliable transportation
