Key Responsibilities- Conduct comprehensive assessments of patients and families to identify their unique needs and develop personalized care plans.
- Initiate and reevaluate patient plans of care as necessary to ensure optimal outcomes.
- Administer medication and treatment as prescribed by physicians and maintain accurate clinical notes.
- Counsel patients and families on meeting their nursing and related needs.
- Communicate effectively with physicians and participate in interdisciplinary group meetings to ensure comprehensive care.
- Ensure compliance with all state and federal regulations and maintain accurate records.
- Graduate of an accredited nursing school with current licensure to practice in the state of operation.
- At least one year of nursing experience, with three or more years preferred.
- Experience in home health or a similar setting is highly desirable.
- Commitment to clinical and documentation excellence.
