Patient Care Pharmacist Job Description

We are seeking a highly skilled and compassionate Patient Care Pharmacist to join our team at Nationwide Children's Hospital. As a key member of our patient care team, you will be responsible for providing expert pharmaceutical care to our patients, collaborating with healthcare providers to optimize medication therapy, and ensuring the safe and effective use of medications.

Responsibilities:
  • Provide direct patient care, including medication therapy management, medication reconciliation, and patient education.
  • Collaborate with healthcare providers to optimize medication therapy and ensure safe and effective use of medications.
  • Participate in multidisciplinary patient care rounds to discuss patient care plans and medication therapy.
  • Develop and implement medication therapy plans, including medication selection, dosing, and monitoring.
  • Provide education and training to healthcare providers, patients, and families on medication use and management.
  • Participate in quality improvement initiatives to improve patient care and outcomes.
Requirements:
  • Doctor of Pharmacy (PharmD) degree from an accredited college of pharmacy.
  • Licensure to practice pharmacy in the state of Ohio.
  • Board certification in pediatric pharmacy or a related field (e.g., pharmacotherapy, clinical pharmacy).
  • At least 3 years of experience in pediatric pharmacy practice, with a focus on clinical care and patient education.
  • Excellent communication and interpersonal skills, with the ability to work effectively with healthcare providers, patients, and families.
