Renal Dialysis Social Worker - Complex Care

Job Summary:

We are seeking a skilled Renal Dialysis Social Worker to join our Complex Care team. The successful candidate will work closely with patients, families, and healthcare providers to develop and implement effective care plans, ensuring optimal outcomes for individuals with renal disease.

Key Responsibilities:

  • Conduct comprehensive assessments of patients' needs and develop individualized care plans.
  • Collaborate with interdisciplinary teams to coordinate care and services.
  • Provide education and support to patients and families on renal disease management and treatment options.
  • Facilitate communication between patients, families, and healthcare providers to ensure seamless care transitions.
  • Monitor and evaluate the effectiveness of care plans, making adjustments as needed.

Requirements:

  • Master's degree in Social Work or related field.
  • Current licensure as a Licensed Certified Social Worker-Clinical (LCSW-C) or Licensed Clinical Professional Counselor (LCPC).
  • Minimum 3 years of experience in social work or a related field, with a focus on renal disease or complex care.
