**Job Overview:**

We are seeking a highly skilled Licensed Practical Nurse (LPN) to join our team at Sayre Christian Village. As a LPN - Skilled Nursing Unit Coordinator, you will play a vital role in ensuring the delivery of high-quality care to our residents in the Skilled Nursing Unit.

Key Responsibilities:

  • Render nursing care in the Skilled Nursing Unit or Dementia Care Unit at Sayre Healthcare Center.
  • Identify individual resident needs, according to dependent and independent nursing functions and conformance with recognized nursing techniques, procedures, and established standards based on the scope of practical nursing.
  • Collaborate with interdisciplinary teams to develop and implement individualized care plans.
  • Administer medications and treatments to residents; monitor the administration of intravenous fluids; place orders for medication from the pharmacy.
  • Maintain accurate and detailed records of resident care and progress.
  • Communicate effectively with residents, family members, and healthcare professionals to ensure seamless care transitions.
  • Participate in quality improvement initiatives to enhance the overall quality of care and service.
  • Other duties as assigned.

Requirements:

  • 2-3 years experience as a licensed nurse in areas including but not limited to Skilled Nursing Facility, Dementia Care, Short Term Rehabilitation, or Orthopedics
  • Active KY Licensed Practical Nurse License.
