About the Role

Kaiser Permanente is seeking a skilled Social Work Professional to join our team in a dynamic and challenging role.

Job Summary

We are looking for a highly motivated and experienced Licensed Clinical Social Worker to provide exceptional care to our members. As a Case Manager, you will be responsible for collaborating with the healthcare team to facilitate the coordination of appropriate services, helping members achieve their optimal level of independence, and enhancing their quality of life.

Key Responsibilities
  • Problem Identification and Assessment: Identify and assess the psychosocial needs of members, including financial counseling and referral, accessing community resources, and guiding members through health-related legal processes.
  • Caseload Management: Effectively manage and coordinate assigned caseloads consistent with established criteria, ensuring timely and cost-effective delivery of services.
  • Plan of Care Development: Develop and monitor comprehensive plans of care in collaboration with the nurse case manager and other healthcare team members, promoting members' optimal level of functioning and enhancing their quality of life.
  • Resource Utilization: Identify, facilitate, and advocate for appropriate organizational and community resources to meet the plan of care, ensuring efficient and timely implementation.
  • Documentation and Analysis: Ensure consistent and reliable documentation of case management activities, analyzing patient and program outcomes to identify improvements in program quality and cost-effectiveness.
  • Community Resource Referral: Facilitate application processes for accessing local, state, and federally funded programs, referring to appropriate community agencies in cases of suspected patient abuse or neglect.
  • Counseling and Education: Provide supportive counseling and education to members, families, caregivers, healthcare team members, and the community, including end-of-life issues and Advanced Directives.
  • Professional Development: Maintain current knowledge of laws, regulations, and policies relating to the practice of social work, adhering to the NASW Code of Ethics.
Requirements
  • Experience: Minimum three years of clinical experience plus two years in case management required.
  • Education: Masters degree in social work (MSW) required.
  • Licensure: Licensed Clinical Social Worker - Certified (Maryland) within 6 months of hire, Licensed Clinical Social Worker (Virginia) within 6 months of hire, and Independent Clinical Social Worker License (District of Columbia) within 6 months of hire.
