GENERAL SUMMARY

Under the guidance of the CMD Manager, the RN Supports Coordinator employs person-centered methodologies to evaluate the medical, functional, psychological, financial, and environmental requirements of MI Choice Medicaid Waiver participants. This role initially requires in-person attendance at our office for training, transitioning to a hybrid model upon successful completion of training and performance benchmarks.

KEY RESPONSIBILITIES

  • Conduct comprehensive in-home evaluations to pinpoint areas of need and service preferences, including the assessment of service frequency and duration as outlined in the care plan.
  • Review and assess participant medications to identify potential contradictions.
  • Analyze disease progressions to collaborate effectively with external entities (e.g., hospice, skilled care, community mental health services) to enhance participant independence.
  • Identify potential health care issues related to nutrition, hydration, continence, and physical conditions.
  • Work in conjunction with physicians, LPNs, and other medical professionals to establish effective treatment plans for participants.
  • Provide educational resources and information to participants and their families regarding home treatment protocols.
  • Collect and synthesize information from diverse sources, including participant self-reports, family feedback, documented medical histories, and assessments from other disciplines.
  • Utilize motivational interviewing techniques to evaluate and articulate the motivations of program participants regarding their identified needs.
  • Manage a caseload to ensure timely completion of assessments, documentation, and reporting.
  • Collaborate with the Community Resources Department to furnish participants and their families with accurate information about available services and supports.
  • Assist in the creation of comprehensive Individualized Person-Centered Service Plans in partnership with participants and their support systems, adhering to Person-Centered Planning principles and Medicaid Guidelines.
  • Conduct in-home reassessments collaboratively as part of an RN/SW Team, ensuring thorough documentation of all service activities.
  • Facilitate connections to community services and supports based on the needs outlined in the Individualized Person-Centered Service Plan.
  • Engage in regular clinical supervision, case consultations, and staff development sessions to optimize professional growth.
  • Adhere to established productivity standards and ensure compliance with program guidelines.
  • Participate in outreach and enrollment initiatives for the Waiver program.
  • Maintain valid state licensure or certification and fulfill all licensure requirements.
  • Uphold professional ethics and boundaries in all interactions.
  • Follow organizational policies and procedures diligently.
  • Perform additional duties as assigned.

EDUCATION AND EXPERIENCE

  • A current Registered Nurse license in the State of Michigan is required.
  • Two years of experience in a hospital, home care, or community-based setting is preferred.
