Job Title: LPN Skilled Nursing Unit Nurse

We are seeking a skilled and dedicated LPN to join our Skilled Nursing team. As a Licensed Practical Nurse, you will play a vital role in providing high-quality patient care and contributing to the success of our healthcare team.

Job Summary:

The LPN Skilled Nursing Unit Nurse will be responsible for providing direct patient care, administering medications, and collaborating with the RN to develop and implement patient care plans. This is a challenging and rewarding role that requires strong communication and interpersonal skills, as well as the ability to work effectively in a fast-paced environment.

Responsibilities:
  • Provide direct patient care, including administering medications, wound care, and vital sign monitoring.
  • Collaborate with the RN to develop and implement patient care plans, ensuring that care is patient-centered and evidence-based.
  • Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
  • Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
  • Respond to emergency situations, such as code blues and falls, in a timely and effective manner.
Requirements:
  • Current IL License and CPR certification.
  • 1+ year of experience in a Skilled Nursing setting.
  • Ability to work every third weekend and holiday.
  • 12-hour shifts, Nights (7p-7:30a).
