Job SummaryThe RN Case Manager will be responsible for coordinating and directing care specific to the needs of each rehabilitation patient. This includes ensuring the implementation of the patient's treatment plan, facilitating patient involvement throughout the rehabilitation process, and documenting findings of discharge planning evaluations and psychosocial assessments.

Three years' clinical, hospital nursing experience required with preference given to those with 2 years of case management experience or 2 years of critical care experience.
