About the Job

We are seeking a skilled Pediatric Registered Nurse Case Manager to join our team at Nascentia Health. This is an exciting opportunity for a registered nurse with experience in pediatric and/or MCH care to work in a fast-paced environment, providing quality care to patients in their homes.

Job Summary

The Pediatric Registered Nurse Case Manager will be responsible for assessing and caring for pediatric clients in Onondaga County, ensuring safe delivery of home health services and developing plans of care based on client assessments and OASIS evaluations. The successful candidate will have excellent communication skills, both written and verbal, and be able to work independently as well as part of a team.

Responsibilities and Standards
  1. Care Delivery and Coordination
  • Ensure quality and safe delivery of home health services, including case managing of client caseloads, developing plans of care, and documenting care in accordance with quality standards.
Documentation and Communication
  • Complete all OASIS evaluations and documentation that reflects nursing care and interactions, ensuring it is clear, concise, and completed in a timely manner.
Team Collaboration
  • Work with direct supervisors, patient care coordinators, and other healthcare professionals to efficiently manage patients using a team approach.
Leadership and Supervision
  • Supervise and oversee paraprofessional staff relevant to patient care, including home health aides, and provide guidance and education on client care.
Requirements and Qualifications
  1. Licensure and Education
  • Current New York State registered professional nurse license (degree from an accredited school of nursing).
Skills and Experience
  • Self-directed and organized with the ability to work independently in a fast-paced environment.
  • Previous pediatric and/or MCH experience preferred.
Physical Requirements
  • Ability to sit or stand at a desk 90% of the day.
  • Ability to perform tasks involving physical activity, including lifting/carrying up to 40 lbs, and pushing/pulling up to 150 lbs.

The salary for this position is $70,131-$81,275 annually, with a competitive benefits package that includes:
