Physical Therapist

As a Physical Therapist with SunCrest Home Health, you will be responsible for providing high-quality physical therapy services to patients in their homes, promoting independence and improving overall health outcomes.

• Assessment and Evaluation: Conduct thorough assessments and evaluations of patients' physical therapy needs, identifying areas for improvement and developing personalized treatment plans.
• Treatment and Intervention: Provide evidence-based physical therapy interventions to patients, including exercises, modalities, and education, to promote optimal health and function.
• Communication and Collaboration: Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination and optimal patient outcomes.
• Documentation and Reporting: Maintain accurate and timely documentation of patient progress, including progress notes and treatment plans.
• Professional Development: Stay current with industry developments and best practices, participating in ongoing education and training to enhance clinical skills and knowledge.

• Licensure: Current Physical Therapy licensure in the state of practice.
• Certification: Current CPR certification.
• Education: Bachelor's degree in Physical Therapy or related field.
• Experience: Minimum 1 year of experience as a Physical Therapist in a home health or geriatric care setting.
