As a Registered Nurse (RN) on our Medical Flex team, you'll have the opportunity to make a meaningful difference in the lives of our patients and their families.
About the RoleWe're seeking a skilled and compassionate RN to join our team of healthcare professionals. As a Medical Flex RN, you'll provide one-on-one patient care and work at the top of your license. You'll be part of an interdisciplinary team that focuses on delivering high-quality, patient-centered care and achieving positive outcomes for our patient population.
Responsibilities- Develop and implement individualized care plans, adjusting as needed to ensure optimal patient outcomes
- Administer medications as prescribed by physicians and prioritize visits for high-risk patients to reduce re-hospitalizations
- Manage multi-disciplinary care, promoting continuity of care through admissions, transfers, and discharges
- Complete all clinical documentation in accordance with agency protocol and Medicare/Federal guidelines
- Collaborate with the interdisciplinary team and physicians to ensure optimal patient care
