About the RoleWe're seeking a skilled and compassionate RN to join our interdisciplinary team. As a Medical Flex RN, you'll provide one-on-one patient care and work at the top of your license. Your primary responsibility will be to develop and implement individualized care plans, adjusting as needed to ensure optimal patient outcomes.
Key Responsibilities- Administer medication as prescribed by the attending physician and prioritize visits for high-risk patients to decrease re-hospitalizations.
- Manage multi-disciplinary care as applicable, promoting continuity of care through appropriate admissions, transfers, and discharges.
- Complete all clinical documentation following agency protocol and Medicare/Federal guidelines.
- Collaborate with the interdisciplinary team and physicians to ensure optimal care is provided.
- Understand and follow agency policies, procedures, rules, and regulations, and communicate changes in schedule/availability to schedulers or supervisors.
We're looking for a motivated and experienced RN with a minimum of one year of experience in an acute care setting. Home care experience is preferred, but not required.
