Key Responsibilities:

  • Collaborate with healthcare providers to develop and implement personalized care plans
  • Coordinate appointments, tests, and treatments, ensuring timely and efficient care delivery
  • Identify and address access-related barriers, including insurance and transportation issues
  • Provide education and support to patients and families, empowering them to make informed decisions about their care
  • Serve as a clinical resource, offering expertise in oncology care management and navigation

Requirements:

  • Bachelor's Degree in Nursing (Required)
  • Master's Degree in Nursing (Preferred)
  • 5 years of experience in nursing, with 2 years in oncology nursing (Required)
  • 1 year of experience as an oncology nurse case manager or patient navigator (Preferred)
  • RN-LIC - PA Registered Nurse License (Required)
  • OCN - OCN - Oncology Cert Nurse (Preferred)
