Job Description

Cordial Health is a leading healthcare organization providing comprehensive mental health services to patients across Yonkers, NY. We are seeking a dedicated and compassionate Social Worker to join our team.


Key Responsibilities:


  • Conduct comprehensive assessments to identify mental health issues, strengths, and treatment needs.
  • Develop and implement personalized treatment plans in collaboration with clients.
  • Provide evidence-based psychotherapy and counseling to address a wide range of mental health concerns.
  • Advocate for clients' rights and assist them in accessing community resources and support systems.
  • Collaborate with a multidisciplinary team of professionals to ensure holistic care for clients.
  • Maintain accurate and timely documentation of client interactions, treatment plans, and progress notes.
  • Participate in regular supervision, case conferences, and continuing education to enhance professional development.

Qualifications:


  • Master's degree in Social Work (MSW) from an accredited program.
  • Current LCSW license in New York.
  • Experience providing direct clinical services in a mental health setting preferred.
  • Strong assessment, diagnostic, and treatment planning skills.
  • Excellent interpersonal and communication skills.
  • Ability to work effectively both independently and as part of a team.
  • Commitment to upholding ethical standards and maintaining client confidentiality.
