Job Summary

We are seeking a compassionate and dedicated LPN to join our team. As a home care LPN, you will provide high-quality patient care in the comfort of their own homes. Your expertise and experience will be invaluable in helping patients maintain their independence and dignity.

Responsibilities
  • Participate in ongoing interdisciplinary assessment of the patient.
  • Develop and evaluate the plan of care in partnership with the patient, representative, and caregiver(s).
  • Provide services that are ordered by the physician as indicated in the plan of care.
  • Recognize and report changes in the patient's condition to the RN.
  • Counsel and educate the patient and family in meeting healthcare related needs.
  • Complete timely and accurate clinical notes including addressing patient's progress.
Requirements
  • Current license as an Licensed Practical Nurse (LPN) in the State of MN. License is active and in good standing.
  • 2 years' experience as an LPN.
  • Able to travel within geographic area serviced by the office. Valid driver's license, reliable transportation, and proof of auto insurance.
  • CPR Certification
