- Current LPN license in the State of MN, active and in good standing
- 2 years of experience as an LPN
- Able to travel within the geographic area serviced by the office, with a valid driver's license, reliable transportation, and proof of auto insurance
- CPR certification
- Participate in ongoing interdisciplinary assessment of the patient
- Develop and evaluate the plan of care in partnership with the patient, representative, and caregiver(s)
- Provide services ordered by the physician as indicated in the plan of care
- Recognize and report changes in the patient's condition to the RN
- Counsel and educate the patient and family in meeting healthcare-related needs
- Complete timely and accurate clinical notes, addressing patient progress
