Job Summary:

Responsibilities:
  • Collaborate with our interdisciplinary team to ensure comprehensive patient care and care planning.
  • Provide ongoing patient observation to determine service frequency and change in Plan of Care in compliance with agency policy and philosophy.
  • Deliver comprehensive nursing care, health teaching, guidance, and supervision to individuals and families at the direction of RNs, physicians, or Medical Directors.
  • Supervise patient care given by Home Health Aides.
  • Develop and maintain health education programs to promote the health and well-being of individuals, families, and communities.
  • Refer patients to other disciplines and community resources as needed.
  • Maintain accurate and detailed patient clinical records, reports, and other written communications according to agency policy and directives.
  • Participate in the development and operation of the agency by contributing to the evaluation of service programs, staff development, and in-service programs.
  • Use your own transportation to provide services in patients' places of residence, adhering to all motor vehicle regulations and laws.
  • Perform IV therapy in accordance with State Practice Act.

Requirements:
  • Graduation from a School of Practical Nursing with a current license as a Practical Nurse.
  • A minimum of two years experience as an LPN.
