We are seeking a highly skilled RN Field Team Supervisor to join our team at Willcare Home & Community Based Services in Middletown, NY. This is a full-time position with an excellent schedule - Monday through Friday 8:30am - 5:00pm (with occasional on-call hours). The successful candidate will be responsible for the coordination of high-quality, cost-effective patient care, as well as the operations and operational flow of the agency.
This role requires a minimum of one year of RN work experience with a Certified Home Health Agency. The ideal candidate will have current RN licensure in the state of practice, a current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation. Additionally, current CPR certification is required.
As a Clinical Operations Manager RN, you will be responsible for the coordination of high-quality, cost-effective patient care, as well as the operations and operational flow of the agency. You will train and direct provisions of nursing and personal care to patients in their homes as prescribed by the physician or as authorized by the state/program and in compliance with applicable laws, regulations, and agency policies.
The successful candidate will be responsible for the following specific job duties:
- Provides high-quality clinical services within the scope of practice and within infection control standards, in accordance with the plan of care/service plan, and in coordination with other members of the patient/client's care team from admit through discharge.
- Completes clinical nursing assessments in accordance with federal and/or state program requirements and as required by payer.

Responsibilities
The Registered Nurse in Home and Community Based Services trains and directs provisions of nursing and personal care to patients in their homes as prescribed by the physician or as authorized by the state/program and in compliance with applicable laws, regulations, and agency policies. The Registered Nurse coordinates the plan of care with other health care professionals involved in care and helps to achieve and maintain continuity of patient care by planning and exchanging information with physician, agency personnel, patient, family, case manager and other community resources.
