About the Role

We are seeking a compassionate and skilled Registered Nurse to join our Hospice team at Foundation Health Partners. As a Hospice Registered Nurse Case Manager, you will play a vital role in providing superior and compassionate care to patients and their loved ones, ensuring they complete their life journey in comfort, dignity, and harmony.

Responsibilities
  • Assess patient physical, psychological, social, spiritual, educational, developmental, cultural, and discharge planning needs.
  • Develop and implement a comprehensive care plan that addresses patient needs, in collaboration with the interdisciplinary team.
  • Provide care based on physician orders and the nursing plan of care, ensuring continuity of care and accurate communication with other caregivers.
  • Evaluate patient and family response to care and teaching, and revise the plan of care as needed.
  • Document assessment, planning, implementation, and evaluation in the patient record, ensuring accurate and timely documentation.
Requirements
  • Current license to practice as a Registered Nurse (RN) in the state of Alaska.
  • One (1) year of professional experience practicing as a Registered Nurse (RN) in a hospice or similar setting preferred.
  • Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN) preferred.

  • $36.42 to $60.33 hourly wage based on experience and education.
  • Retention Bonus: $10,000 - $15,000 (DOE).
