Full-Time Wound Care Nurse (Monday to Friday, on-call weekends once a month)

Position Overview
In alignment with our mission to enhance the quality of life for our Residents, the Wound/Treatment Nurse is essential in delivering outstanding nursing care and customer service. This role involves executing skin treatments for all Residents under care, obtaining treatment orders from attending physicians, and adjusting treatment plans in accordance with established protocols. The Treatment Nurse will oversee staff and ensure the health, safety, and welfare of all Guests by adhering to relevant laws, regulations, and nursing standards.

Key Responsibilities

Daily Duties

Conduct comprehensive assessments on all new admissions and re-admissions to identify any open areas as noted by the admitting nurse. Ensure that skin assessments, treatment orders, and preventative measures are documented and implemented.

- Monitor residents with new skin alterations, ensuring that skin assessments, treatment plans, and care plans are updated accordingly.

Weekly Responsibilities

- Update skin assessments weekly, ensuring completion by the end of the designated day.

- Unit Managers to revise care plans weekly for pressure and non-pressure areas, with updates due by Fridays.

- Submit a Wound Report to the VP of Clinical Services by Thursday detailing the percentage of Center Acquired Pressure Ulcers, aiming for a target of 3% or less.

- Administer treatments for vascular, surgical, and pressure wounds of stage 2 or greater from Monday to Friday.

- Charge Nurses to manage stage 1 skin tears, ensuring weekly skin assessments are completed.

Ongoing Responsibilities

- The Wound Nurse is tasked with preventing wounds through daily staff education and reminders regarding resident care. This proactive approach is vital for effective wound management.

- Adjust treatment plans within a two-week timeframe for any worsening wounds, in consultation with the Wound physician and PCP.

- Implement preventative measures for residents at risk of developing Center Acquired Pressure Ulcers, including T&P orders and appropriate barrier creams.

Additional Duties

1. Collaborate with the nursing team to assess and develop skin care treatment plans.

2. Initiate referrals as necessary.

3. Evaluate residents upon admission to distinguish between normal and abnormal findings, referring to physicians when needed.

4. May serve as a Unit Nurse when required.

5. Supervise C.N.A.s and subordinate staff, ensuring task completion.

6. Administer medications within the scope of RN or LPN licensure and maintain accurate narcotic records.

7. Document care contemporaneously during shifts and provide detailed monthly evaluations of resident progress.

8. Ensure adherence to residents' care plans and assess their status accordingly.

9. Participate in weekly rounds with the Wound physician.

10. Stay informed of individual care plans and support the care planning process by reporting relevant observations.

11. Provide wound care as needed and delegate tasks appropriately.

12. Complete weekly skin assessments.

13. Maintain infection control and safety practices during care delivery.

14. Uphold the comfort, privacy, and dignity of residents, fostering a caring environment.

15. Respond promptly to resident call lights.

16. Assist with new admissions and re-admissions as directed.

17. Communicate effectively with residents, families, peers, and supervisors.

18. Attend departmental meetings and training sessions as required.

Qualifications

  • Graduation from an accredited nursing program.
  • Current State License as a Registered Nurse or Licensed Practical Nurse.
  • C.P.R. Certification.
