Job Title: Licensed Practical Nurse

Job Summary:

We are seeking a skilled Licensed Practical Nurse to join our team. As a Licensed Practical Nurse, you will be responsible for providing high-quality patient care and working collaboratively with our healthcare team to ensure the best possible outcomes for our patients.

Responsibilities:
  • Provide 1:1 care for clients, following their individualized plans of care and maintaining their dignity and independence.
  • Monitor clients' vital statistics and report any abnormal findings to the case manager and physician.
  • Administer medications, feedings, oxygen, and ostomy care as needed, and document the results of these interventions.
  • Complete routines established by other care professionals, such as Physical Therapists, Occupational Therapists, and Speech Therapists.
  • Ensure infection control policies are maintained at all times and report any possible communicable diseases or infections.
  • Accurately document nursing actions and communicate with family, case managers, and physicians, as well as document progress and outcomes for established goals.
  • Order supplies and medications as needed to ensure adequate amounts are on hand.
  • Ensure all patient needs are being met in accordance with physician instructions.
  • Monitor incidents involving individuals and complete incident reports in a timely and accurate manner.
  • Ensure all medical equipment is maintained and report any faulty equipment to Durable Medical Equipment for repair.
Qualifications:
  • Completed post-secondary coursework to facilitate licensure as a Licensed Practical Nurse or Licensed Vocational Nurse.
  • Knowledge of individual service plans, including understanding of client rights and health needs.
  • Proof of current license as a Licensed Practical Nurse or Licensed Vocational Nurse in good standing from the state(s) of practice.
  • CPR certification.
  • Excellent interpersonal skills, including customer service, patience, empathy, and compassion.
  • In-depth understanding of patient care policies and regulations.
  • Familiarity with symptoms and signs of improvement for several medical conditions.
