LPN - Home Care Specialist

Constellation Health Services is seeking a skilled Licensed Practical Nurse (LPN) to join our Home Health team. As an LPN, you will provide high-quality patient care in the comfort of their own homes, working collaboratively with our interdisciplinary team to achieve positive patient outcomes.
Responsibilities- Provide comprehensive nursing care to patients in their homes, including assessments, treatments, and education.
- Develop and implement individualized care plans in collaboration with the patient, family, and healthcare team.
- Conduct regular home visits to monitor patient progress, provide support, and make adjustments to care plans as needed.
- Collaborate with other healthcare professionals, including RNs, therapists, and social workers, to ensure seamless care coordination.
- Document patient information accurately and thoroughly, maintaining confidentiality and adhering to HIPAA guidelines.
- Participate in ongoing education and training to stay current on best practices, new technologies, and regulatory requirements.
- Graduate of an accredited LPN program.
- Current licensure as a Licensed Practical Nurse in the state of New York.
- Minimum of 1 year of clinical experience in home health or a related field.
- Ability to work independently and as part of a team, with excellent communication and interpersonal skills.
- Strong problem-solving and critical thinking skills, with the ability to prioritize tasks and manage time effectively.
