Job Title: Licensed Practical NurseJob Summary:

We are seeking a skilled Licensed Practical Nurse to join our Home Health Program. As an LPN, you will provide high-quality patient care in a community setting, working collaboratively with our interdisciplinary team to achieve positive patient outcomes.

Responsibilities:
  • Provide nursing care as defined in the State Nurse Practice Act, including specialized duties related to end-of-life care.
  • Implement current nursing practice following a comprehensive assessment and the plan of care.
  • Maintain accurate and timely documentation of patient services to reflect the plan of care.
  • Assess and provide patient and family/caregiver education and information pertinent to diagnosis and plan of care.
  • Recognize and interpret symptoms and institute remedial measures within the scope and limits of a Licensed Practical Nurse.
  • Observe, record, and report to the immediate supervisor any changes in the patient's condition.
  • Participate in the coordination of hospice and other home care services, appropriately reporting identified needs to the interdisciplinary group.
  • Participate in Hospice IDT meetings.
  • Implement care to achieve outcomes of comfortable and safe dying and effective grieving for the patient and family.
  • Use and prepare equipment and supplies effectively, adhering to aseptic technique.
  • Provide appropriate pain/symptom management. Evaluate the patient's response to treatments/medications.
  • Participate in personal, professional growth and development. Participate in the organization's quality assessment and performance improvement team activities.
  • Perform other duties as assigned by the registered nurse or clinical supervisor.
