About the Role:
We are seeking a dedicated Palliative Care Nurse Practitioner to join our team at Northeast Healthcare Recruitment. This position focuses on symptom management, chronic disease management, goals of care, and advanced care planning.
Key Responsibilities:
- Provide 100% inpatient care, rounding on inpatient hospice patients Monday through Friday.
- Manage an average of six patients per day, with hospitalists handling admissions.
- Work in an open ICU setting.
Requirements:
- Board Certified Nurse Practitioner (NP)
- Geriatric Certification or willingness to obtain within one year
- Active Ohio license or willingness to obtain
- Minimum of three years of experience as an NP in an acute care setting, or with Palliative Care and Hospice
