About the Position

We're seeking a compassionate and skilled Nurse Practitioner to join our team, providing primary, urgent, and acute medical care to a wide variety of patients in their homes. As a key member of our care community, you'll work closely with our physicians, social workers, paramedics, and care team coordinators to deliver the highest quality care to our patients.

Responsibilities
  • Provide primary care to patients in the home, including initial screenings, physical examinations, and assessments
  • Facilitate staff, patient, and family decision making by providing educational resources
  • Collaborate with community teams to complete care assessments and develop care plans
  • Order, interpret, and analyze diagnostic tests, and determine the need for follow-up appointments and further treatment options
  • Prescribe and manage medications and treatments as appropriate
  • Update patient records and ensure accuracy at each patient appointment
Requirements
  • Current licensure and certification as a Nurse Practitioner
  • Experience working with geriatric patients and their families
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
